We are one of the very few medical offices in the United
States specializing in vein therapy exclusively. All work is done
by Dr. Martin Schulman or Dr. Lee Schulman, in contrast to other
offices where nurses or technicians often treat patients. It is
possible to treat all women with injection therapy regardless
of vein size, and it is only the rare male patient who requires
surgery. We have also started to use special, newly FDA approved
Endo-Venous Lasers to treat large veins. Our most grateful patients
are those who have had vein surgery in the past, have developed
other veins, and are then treated with a simpler and safer method.
Injections
are done using the smallest and finest needles available. Injection
sites are covered with a cotton pad and surgical tape. We do all
therapy in our offices on an outpatient basis and patients are
able to resume normal activity immediately. All medicines are
FDA approved and have been used for more than 50 years, and all
needles and syringes are single use and disposable. You will be
phoned the day after your first visit, both as a courtesy and
to answer any questions you may have. Some patients are concerned about sun exposure after vein treatment. The truth is that you can return to
normal activity, including sun exposure, after
treatments. Remember this type of work is done
routinely in sunny areas such as Florida and
California and the results are fine, but as always sun
block is recommended.
Patients
frequently comment that removing their tapes, which is like taking
off a long band-aid, is the most uncomfortable part of the procedure.

Treated
veins don’t go away immediately, although your symptoms
will usually improve while the veins are healing. We often compare
leg vein therapy to the treatment of a broken bone; it can be
“fixed” but it takes time for your body to do its
part and heal the area. Our advanced techniques enable all patients

What
Are Varicose And Spider Veins?
Varicose
veins are the large ropey veins seen in the legs and spider veins
are smaller red or blue lines. A vein becomes varicose when the
valves in its wall malfunction and fail to help bring blood back
to the heart. Veins are thinner and much weaker than arteries,
and they may enlarge in response to pressure or, more commonly,
or as a result of a genetic predisposition. Untreated varicose
veins only worsen: the increased pressure in enlarged veins results
in further enlargement.
Spider
veins are the thread-like veins seen on the surface of the skin.
They are usually less painful than varicosities, but without treatment
may bleed or worsen. Although most commonly seen in the legs,
spider veins may be present on the face and other parts of the
body. These spider veins, medically referred to as telangectasias,
do not become bulging varicose veins, but may progress locally
and become a cosmetic problem. Ironically, because they are on
the surface of the skin, small veins may take longer
to heal (disappear) then large veins.
Why
Treat These Veins?
You
may visit the Schulman Vein and Laser Center for cosmetic reasons
or to realize the medical benefits of treatment. As we have said,
without treatment your veins will only enlarge and worsen. Typical
symptoms include heaviness, tiredness, night cramps, swelling
(edema), aching, throbbing, and burning. These symptoms often
worsen as the veins enlarge. Many patients remember their parents’
or grandparents’ legs and want to start therapy before they
get to “that point.”
Untreated
varicose and spider veins may lead to other medical problems.
As they enlarge, venous insufficiency, which is a pooling of venous
blood that is unable to return from the leg to the heart, may
occur. Venous insufficiency itself leads to other complications,
the most common of which are blood clots (phlebitis), ulcers (breaks
in the skin), bleeding, infection and stasis changes (a “rusty”
discoloration) of the leg. We treat patients with phlebitis, bleeding,
or ulceration for their acute problems. When the acute condition
is resolved (for example, an ulcer has healed), it is important
to treat your varicosities and prevent future problems.
How
To Treat Varicose And Spider Veins?
The
old saying “there are many ways to skin a cat” applies
to the various methods of therapy available for your venous problems.
One
conservative treatment is the wearing of support stockings (to
be put on before getting out of bed in the morning and worn all
day). While support hose will NOT make the veins go away or prevent
them from worsening, they may provide symptomatic relief. We believe
treating the veins directly is far preferable to committing to
a lifetime of wearing unattractive and cumbersome stockings.
Injection
of medication into veins is known as SCLEROTHERAPY. It has been
a successful method of treatment for more than a century. At our
offices, all veins are treated by sclerotherapy, with excellent
results. The treatment causes the vein walls to gradually seal
shut and disappear. It is equally
effective for all veins, from the largest varicosities to the
smallest spider veins.
SCLEROTHERAPY
IS, WITHOUT QUESTION, THE SAFEST, SIMPLEST, AND MOST EFFECTIVE
METHOD OF TREATING UNWANTED VEINS. It is minimally invasive and
virtually painless. When sclerotherapy is done on an outpatient
basis, anesthesia is not necessary, and you may resume normal
activity immediately.
New
FDA approved Endo-Venous Lasers are our newest tool in the treatment
of large veins. The procedure of laser therapy is done in our
offices and there is no need for general anesthesia. Shortly after
the completion of the procedure, patients are able to leave the
office on their own and without difficulty.
How Does Therapy Help?
When
malfunctioning veins are eliminated, blood is forced into normal
blood vessels and returns to the heart efficiently. Your circulation
is actually
improved, since blood that was pooled in varicose and spider veins
is no longer stationary and stagnant. After vein treatment, patients
whose symptoms were due to venous congestion and insufficiency
experience
great relief: without stagnant blood filling their varicosities,
their legs feel less heavy and tired.
Patients
frequently ask if they will have “enough” veins left
after treatment. It is important to understand that the veins
we treat are abnormal and are actually harming the circulation.
